Fall Conference Registration Application

Instructions:
Please feel free to print these instructions before you proceed.
IT IS IMPARATIVE you remember / do the following when registering;

1. | If you are an office secretary and will be registering someone in your office, please use the person’s name that will be attending the conference and NOT your name. You can however use your email in order to receive the automated confirmations. |

2. | If you are registering for more than one person;
|
When you click the “Check Out” button at the bottom of the order form, you will be taken to a select payment screen. Click on your preferred payment method then click continue. A “Final Confirmation” screen will come up. DO NOT click the first “Continue / Place Order” button you see. At the bottom of the Final Confirmation screen there is a comment box. In this box you will need to enter the names of the people you are registering for. If you will exceed 250 characters you will need to email me separate list of names referencing your order number, name and organization. jd.mitchell@vfpa.org |

3. | On the "checkout screen," please make sure you include your locality / jurisdiction in the "organization" field. |
| 4. | If you want to use PayPal and have a government issued credit card, you will need the credit card billing info in order to get PayPal to accept your credit card. There is still no guarantee even with that information PayPal will accept your card. |
| 5. | IMPORTANT!!! On the following order form, there is a space that asks if you or the person you are registering for will be eating lunch. Lunch is included in the conference fee. Please be sure to include if you or the people you are registering for will be eating lunch. This will help provide us with a good head count so we do not waste money on food that no one will be there to eat. |
| 6. | EVEN MORE IMPORTANT!!! On Tuesday, November 8th we are running two tracks; a Technical Track, Sprinkler System Plan Reviews and Blasting and a Management Track, Vision 20/20. When registering, YOU MUST CHOOSE ONE OR THE OTHER. Vision 20/20 is limited to the first 50 people that register for that track. Pay clase attention during checkout which classes you are registering for. |
